Died at 4:00 a.m. on 18 January 1930 at "Luthern [sic] Hosp," at St. Louis, Missouri at the age of 57 years, 10 months, 29 days. His usual residence was 1395 Arlington Avenue, Ward 6 [St. Louis, Missouri is implied?]. He was single at the time of death. His usual occupation was farmer. Cause of death was "Pulmonary embolism" (1 hour duration). From the supplemental page: "Primary seat in the Bladder. Information given over Phone by Dr H. L [sic] Lund." Contributory cause of death was "[Vesical?] carsinoma [sic]" (6 months duration). From the supplemental page: "Vesical Carcinoma." Disease was contracted at Treloar, Missouri. There was an operation prior to death on 11 January. There was no autopsy. Diagnosis was confirmed by "Section of tumor." Burial, cremation, or removal was to be on 19 January 1930 at or to Treloar, Missouri. The undertaker was Hy. Leidner [Und?] Company, of 1*17 N. Market Street. Personal information was provided by [N?] F. Oberhelman, of 1395 Arlington Avenue. Herbert G [L****?], M.D., of 600 Carleton Building, had attended the deceased from 6 January 1930 to 18 January 1930 and signed the death certificate. The certificate was received by the local registrar on 20 January 1930.
